Commons Board Election Forms

Please choose the correct form to place your vote. You can print and mail these forms to Burlington Property Services at the address on the form or you can fill them out and email them to: caroline@burlprop.com

This form is used if you are the homeowner and are choosing to cast a vote in the election.

This form is to be used to designate a proxy to vote on your behalf. That person must be a member of the community in good standing.

This form is to be used to cast your vote if you have been designated as a proxy by another community homeowner in good standing.
